REFERRAL PROCESS
Family Home Health Services provides personalized medical
services in the familiar and comfortable surroundings of the patients
home.
Recent hospitalization is not required. However, if you
have been hospitalized, planning for your care at home begins before you
leave the hospital to ensure the smooth transition and continuity of quality
care.
• If the Patient is In the Hospital
1. The patient or family member should consult with their physician to
determine if home care is appropriate for their specific needs.
2. If home care is prescribed by the physician, the patient or a family
member may request Family Home Health Services, if this is their agency
of choice. Every patient has the right to choose the home care service
they desire. If you do not specify a particular agency, such as Family
Home Health Services, the physician or hospital social worker can randomly
choose a home care service for you.
3. When the patient is discharged from the hospital the patient or family
should verify that a home care order has been written and, if Family Home
Health Services has been requested, that information is on the order.
4. Upon receipt of the physicians orders, Family Home Health Services
will coordinate with the patient or family to begin delivery of home care
services.
• If the Patient is at Home (No Hospital Stay)
1. Call the family physician and make an appointment for the patient to
see the physician.
2. If the physician determines that home care is appropriate, ask the
physician to call Family Home Health Services, toll free 1-800-246-0084,
and ask for Central Intake to make a referral.
3. If the physician is not sure the patient qualifies for home care services,
Family Home Health Services can provide an evaluation visit to determine
eligibility.
